We are committed to providing high-quality, safe and affordable housing for the community. We specialise in supported living for vulnerable adults and complex needs cases, achieved through a person-centred, trauma informed approach to support.

Our Story:
Transitions Housing Solutions CIC was founded in 2020 during the COVID-19 pandemic to help address the rising levels of homelessness in our communities.
Seeing the challenges faced by friends, family, and local residents inspired us to create a better way to support people into stable housing.
Transitions Property Management Ltd was established to drive growth and expand our supported housing portfolio across the region (and beyond!).
Our journey now continues as we grow responsibly while staying true to our mission: helping vulnerable people access safe, supportive homes.
